Tax Form Instructions

*** Tax receipts from the bookstore may not have your tax charges broken up for you. Total Tax is 7% of your purchase

Example
Total Sales: $400
$400 x 5% = $20 (state sales tax rate)
$400 x 1% = $4 (Local Option Tax)
$400 x 1% = $4 (School Local Option Tax)

Sales Tax Form (5% IA 843 Claim for Refund)

  1. Download and print the tax exemption form.
  2. Fill in your name, campus address, and Social Security Number.
  3. Leave the following two sections blank: "Sales or Use Tax Permit Number" and "Federal Identification Number."
  4. For "County Number" write 85.
  5. Check "Retail Sales Tax."
  6. For "Claim Period" fill in the fiscal quarters and years. For example Books claimed from August 28, 2000 to January 8, 2001 would be filled in "3rd of 2000- 1st of 2001". (Books bought from Aug.-Sept. are 3rd quarter, Books purchase in January are 1st quarter)
  7. In the table, under the "Original Tax Paid" column, write 5% of your purchase, the amount you paid for state sales tax (A $20 purchase would enter $1.00).
  8. Fill out the "Tax Period". Defined below:
    January - March = Period 1 = 03-31-yy
    April - June = Period 2 = 06-30-yy
    July - September = Period 3 = 09-30-yy
    October - December = Period 4 = 12-31-yy

    Example: Textbooks purchased in August = 09-30-yy (09 being september, or the end of the period in which they were purchased). Textbooks purchased in January would be the first quarter written 03-31-yy.
  9. For "Reason for Refund Request" just write "UBS at ISU."
  10. Sign the bottom of the form.

Local Option Sales Tax Form (Computation of Local Option Sales Tax)

  1. When filling out the local option sales tax form, list the same period used in step 8.
  2. The county number is 85 for Story County.
  3. Under "Original Tax Paid", list the total of the 1% local option tax (L/O Tax), and the 1% school local option tax (SiLO tax). (eg: a $20 purchase would add $0.40, $0.20 for each tax)
  4. Leave "Corrected Amount" blank.
  5. L/O Tax to be Refunded should be 1% of the purchase, or the amount covered only by the L/O Tax. (a $20 purchase would enter $0.20 here)
  6. SiLo Tax to be Refunded should also be 1% of the purchase.
  7. Attach the original receipt to the forms and mail them to:
